Parameters for PEMF therapy
Much of PEMF use is in the experimental domain today. There are some obvious choices, and optimization of treatment parameters specific to individuals and conditions is always possible. We can broadly consider 2 main treatment factors – frequency and amplitude/intensity. The other factors would include the size/area of the field and individual conditions. Certain frequencies at certain intensities would suit some cells better than others.
A NASA study in 2008, shows a 0.01 – 0.1 Gauss powered 10 Hz PEMF modulates more 10,000 genes in neuronal cells. “Specific genes such as human germline oligomeric matrix protein, prostaglandin endoperoxide synthase 2, early growth response protein 1, and insulin-like growth factor binding protein 3 precursor are highly up-regulated. Keratin Type II cytoskelatal7, mytotic kinesin like protein 1, transcription factor 6 like 1, mytotic feedback 26 control protein, and cellular retinoic acid binding protein are down-regulated.
Each of these two sets is only an example from the approximately 320 genes changes expressed as a consequence of exposure to the field.” As this is a cellular level study, an in-vivo (human) study would aim to deliver the intensity to the target. As per the Coulomb's Inverse Square Law of Magnetism, the intensity of a magnetic field drops by the square of the distance, therefore for applying the PEMF field at a desired intensity in a tissue inside the body, an appropriate intensity and distance calculation would need to be made.
To put it simply a 1000 Gauss field would drop to less than 10 Gauss if its placed about 6 inches away from the body. So whatever the intensity of the magnetic field your device generates, much lesser is actually delivered to cells internally. Thankfully, the body does not need much, the schumann resonance’s intensity is less than 0.1 Gauss, the Earth’s magnetic field intensity is less than a Gauss and heart generates EMFs in nanoTeslas.
Most amount of PEMF research utilizes lower frequencies, usually below 50 Hz. Of these I find the frequencies and intensities that fall within the Schumann resonance range (> 25 Hz) and the intensity of Earth’s Magnetic Field (>1 Gauss) are easier on the body as the body is used to them.
Although low-frequency PEMFs below 15 Hz will generally have a positive effect on target tissues, further research shows treatment for different conditions might require different frequencies and intensities for best results. Also some frequencies and intensity combinations will work better for different molecular mechanisms and cell types. Neuronal cells for example display most energetic response at 10 Hz, and the response drops by 50% by moving to 5 and 15 Hz.
Particularly 10 Hz has been found to be conducive in supporting neuronal cells. Similarly the tight lower band of 0.1 – 10 Hz has been studied for use in treatment of rheumatoid arthritis. We also have higher frequency studies at 50 Hz and 75 HZ, even 100 Hz these days which show benefits but it is difficult to ascertain if the benefits outweigh the use of other parameters at lower frequencies.
To decide the treatment parameters, it is important to note the frequency and intensity utilized by reference clinical trials. This will help the practitioner get closer to choosing the right equipment and desired outcomes.
It’s important to note that not all PEMF clinical trials conclude that the PEMF treatment had health benefits or achieved the desired outcomes. As the “dosage” of PEMF field is subjective to factors such as treatment time, frequency and intensity, such studies should be further investigated for these factors before knowing for sure whether PEMF can really help or not. For example, some athletes may be using PEMF regularly up to 7 days a week for several years, while others might just be utilizing treatments sporadically before and after events or workouts. The long-term and short-term effects of these applications would also vary depending on the intensity and frequency of the PEMF.
It’s possible to achieve different outcomes with different treatment parameters, however there are some parameters which are commonly found among PEMF systems today. There is still a lot of work remaining in devising treatment parameters, more comparative studies are needed. As the technology has been under research from a long time now, some treatment frequencies such as 10 Hz are widely accepted to regenerative.
